www.justbecroydon.org is a site operated in partnership by Social Change UK and the Active Lifestyles team, District Centres & Regeneration within Croydon Council.

Use of the "MOT's" within Just Be

The Health MOT and Mind MOT used within the Just Be Croydon website are designed for Croydon residents aged 16 years and over. The Health MOT and Mind MOT are not medical assessments, they are designed to screen eligibility to a one-to-one service (Health MOT only) and to provide some general direction relating to the health and wellbeing of the user. If you are concerned about your health you should speak to a healthcare professional.

The data you supply to the "MOT's" will be captured, held and reported upon. For further information regarding the use of this data please see the Privacy Policy for this website.

If you do not wish for your personal information supplied to the MOT's to be processed (in line with the General Data Protection Regulations 2018) then you should not start and subsequently complete either of the MOT's.

By completing the Health MOT you may be deemed eligible for a one-to-one service "Just Live Well". This service is managed by Croydon Council and utilises a range of external commissioned providers for delivery. The results of your Health MOT will be forwarded to a data management system (PharmOutcomes) for processing. In order to enable this process you will be provided a briefing regarding the Just Live Well service and asked to provide your name and either a telephone number or email address. You are able to opt out of the one-to-one service at this point. Your ongoing involvement with "Just Live Well" will be captured by PharmOutcomes (not this website). All data you supply to "Just Live Well" will be anonymised for reporting purposes to limit the ability for patient identifiable data to be reported on.

We are the owner/licensee of all of the intellectual property on our site and all material published on it. These are protected by copyright laws. All rights are reserved.

You may print off or download extracts of any of our pages from the site for personal use only or draw attention of others to content posted on our site. You may not modify any materials downloaded or printed from the site and you must not use any materials such as photographs, videos, audio or graphics and these must remain with the accompanying text.

Our status (and that of any identified contributors) as the authors of content on our site must always be acknowledged. You must not use any part of the content on our site for commercial purposes without obtaining a license to do so from us or our licensors.
